Case Manager
Position Summary:
Seeking a qualified Case Manager for our veteran support programs. Case Managers must be willing to work with veterans and family members having a wide variety of needs.
Client Assessment: Conduct initial assessments to understand a client's needs, their strengths, and their goals.
Individual Plan Development: Create individualized success plans to include specific needs, goals, timelines, and strategies to address all needs.
Resource Identification and Application: Find and connect clients with appropriate services based on needs, which may include employment, education, childcare, healthcare, insurance, burial information, and financial assistance.
Service Coordination: Work with other service providers, community, local, federal, and state agencies to coordinate appropriate support.
Monitoring and Evaluation: Track client progress, adjust individual success plans as needed, and evaluate the effectiveness of all services provided.
Advocacy: Support clients' needs and rights as a veteran and or family member of a veteran.
Documentation: Maintain accurate running records of client interactions and progress.
Job Type: Full-time
Pay: From $20 per hour
Location: In Person

Requirements
Valid driver’s license required.
Mentoring, coaching or training experience
Outstanding organizational skills
Bachelor’s Degree required.
Professional manner and appearance
Strong oral and written communication skills
Ability to work independently and within a team
Ability to work effectively with diverse group of clients, staff, and community members
High Computer Competency
Preferences
Certification in Substance Abuse Counseling preferred
At least one year of experience in social services or related fields preferred
Experience working with veterans preferred
Degree in Social Work, Psychology, Counseling, or other related social services fields preferred
